Cirrhose décompensée et ascite
Algorithme et points pivots de Dr H. Omni, construits à partir des références citées et inspirés du schéma « Cirrhosis · Ascites » des Clinical Problem Solvers, qui n’est pas reproduit.
🧭 Algorithme
Points pivots
- Ascite : ponction diagnostique chez tout patient admis ; le GASA (albumine sérum − ascite) ≥ 11 g/L oriente vers l’hypertension portale, < 11 vers péritonite, cancer, pancréas.
- Neutrophiles ≥ 250/mm³ dans l’ascite = péritonite bactérienne spontanée.
- Toute décompensation (ascite, encéphalopathie, saignement, ictère, IRA) : chercher le déclencheur (infection, saignement, alcool, médicaments, hépatite, thrombose porte, CHC).
🚩 À ne pas manquer
- Péritonite bactérienne spontanée.
- Hémorragie variqueuse.
- Syndrome hépatorénal.
